What was the lighting situation in the area?
for example, my situation was in a dark room,
BUT there is a street light right outside the
window that shined in the room.

How old were you when the apparition was witnessed?

You could ask about locations… inside, outside, limestone buildings, water, wells, ect… And do they know of any legends/history of the area that might explain the incident-old graveyard, ect… Were they renovating the building? Any other recent work done--major plumbing, electric, digging, landscaping, anything that changed the environment.

Tom, here are some questions from a book that I just bought: Picture yourself ghost hunting.
Address of site
how many occupants at location
occupant's names and ages
occupants religious beliefs
occupants occupations
time of occupancy at the location.
age of the site
how many previous owners
history of the site (tragedies, deaths, previous complaints)
how many rooms in the site
has the location been blessed
has there been any recent remodeling
any occupants on medications
any occupants using illegal drugs
any occupants drink alcohol heavily
any occupants interested in the occult
any occupants seeing a psychiatrist
have any religious clergy been consulted
has there been any media involvement
have there been any other witnesses besides the occupants
have there been any odors: (perfumes, flowers, sulfur, excrement)
have there been any sounds: (footsteps, knocks, banging)
have there been any voices: (whispering, yelling, crying, speaking)
has there been any movement of objects
have there been any levitation
have there been any uncommon cold or hot spots
have there been any problems with electrical appliances
have there been any problems with plumbing
any occupants having nightmares or trouble sleeping
have there been any physical attacks
are pets affected
when was the first occurrence of the phenomena
what were the witnesses' reaction during the phenomena
what was the duration of the phenomena
who first witnessed the phenomena
were there any other witnesses
what time was the first occurrence of the phenomena
how often does the phenomena occur
do the occupants feel the phenomena is threatening
what do the occupants believe is happening: (is it supernatural)
do all of the occupants agree on what is happening or do they think it's non-sense
Actually that's all the questions they list.

Id ask them to describe their general mindset/state and what they were doing prior to the incident. Have they been under any emotional stress?

I have a couple of questions like that, Dbunker, I like those types of questions. Personally, I think a lot of what is 'witnessed' falls into this type of category… Thanks though! It's a great topic for discussion, too.

Does the apparition appear regularly or was it a one time occurrence?

Another question pertaining to the person who saw the "ghost":
At the time they saw the "ghost", were they suffering from a headache/migraine, flu, etc?
I tend to be more "sensitive" to paranormal energy when I'm under the weather, and I was involved once with a study where the intent was to try to correlate sensitives who suffer from migraines more often than the normal person, without any other logical explanation, to the ability to see, hear and sense ghosts. It was quite interesting and made a lot of sense, actually.
Also, how often have they seen ghosts, or do they see them often? Have they seen them since they were a child? Have they seen the same ghost and if so, approximately how many times, at what locations and for how many days/weeks/months/years?

Other possible factors:
time of day,
time of the month with regards to lunar cycles,
time of year,
what was the weather like,
humid or dry,
seismic activity in the area,
construction in the area,
and so on.
