I am hoping I can get a little help on this project that I am doing. I am creating a questionnaire that will eventually be compiled into a data base; it's only for those people who have claimed they have seen an apparition. It's simple stuff really, but I am looking for as many types of questions as I can, and was hoping some people might chime in – for example:
1.) How would you describe the apparition?
a. full-bodied
b. partial
c. shadow
2.) Would you consider the apparition to be:
a. threatening
b. non-threatening
c. unsure
…you get the gist. I'm looking for as many variables as possible – I've come up with about 15 so far. Can anyone think of some questions you'd ask someone about seeing an apparition? /unsure.png' class='bbc_emoticon' alt='

had you been drinking or taking any kind of medication prior to seeing the apparition?
(not a snarky question, i really think it's important to ask)

What did the apparition look like?
Was there any communication or interaction with the witness.
Where did the incident take place?
How many times?
Was the apparition of a relative or friend?
Day or night?
Did they see it upon waking from sleep? Or was it while they were fully awake and not in bed?

Some that I've read on other questionnaires were:
Has your house ever been blessed?
What religion are you?
Has your house ever been exposed to deadly gases like radon or carbon monoxide?
I'll post the others when I find that book.

but seriously i suppose you could ask if the apparition was of a person or an animal.
i dont believe in animal ghosts, but i know some people do.

I think this question could lend some insight into the observers state of mind?

I'll eventually be breaking this survey into four sections, and one section will be based more on the witnesses' state of mind/condition. So that's added – thanks!

Well, taking into account that the word apparition is the act of appearing and not necessarily what you see. I would first ask questions in that regard.
Meaning how did they see it rather than what did they see.
How did it form?
Was it transparent or translucent?
Was it defined or wispy?
Did they see it head on or out of the corner of their eye?
Then
Did it linger?
Did it disappear instantly or fade away?
Kinds of things along those lines.

Exactly – that's why the survey is about it's appearance more than anything else. I want to be able to compile data about when an apparition first forms, but also about the person's reaction, and what other details (i.e. time of day, weather, etc).
I like 'How did it form' I don't have that one yet! Also how it went away…
